Understanding Person Centered Planning: A Holistic Approach to Individualized Care

Every participant that receives services at Gravelbourg Bon Ami, Inc. has a Person-Centered Plan (PCP). This document is the guide to each individual and outlines the way to communicate with the individual, their hopes and dreams, their medical background, and more! The most important goal with a Person-Centered Plan is to ensure that the participants are in charge of their own lives, and to make sure that the Agency is supporting them in the way THEY want to be supported.

Participants decide what their goals are, who they want to help them and how they are going to achieve their goals. The participant decides how they want to be supported in the goal, and who is important in helping them achieve it.


Each participant has different goals - just like you. There is no "One Size Fits All" approach to caring for individuals. Each plan is completely catered to the participant, and each goal in their plan is their own. For some, it is important to them to learn skills about how to live independently. They may have goals about learning to cook or clean. For others, relationship development and maintenance is a priority, and their goals reflect how they plan to achieve those relationships.


Throughout the year, each interaction and activity with the participant is planned with their goals in mind.


The goals are not set in stone either. Like with each of us, sometimes our best intentions don't come true. How many times have you made a New Years Resolution, only to change your mind or forget about it a few months into the New Year? This is a reality for individuals at our agency too. The Person-Centered Planning documents are referred to as "Living Documents" and can be changed at any time throughout the year to ensure they reflect the participants true desires.

In the past, society viewed people who experience intellectual disability as "less than" or "sick" and concentrated all their energy on how to "fix" them. Over the years, the disability sector has changed directions, and instead of looking to fix people, we have been dedicated to working FOR people with intellectual disabilities and helping them to achieve the life they truly desire.


For each person's plan, they choose a "Core Group" of people to help support them in their plans. This might include friends, family, service providers, Social Workers, and other members of the community. The participant decides who they would like to attend their meeting, and everyone present at the planning meeting is there with the intention of supporting the person to live the life they want.
